Is the Abortion Pill Safe?

Yes. Medication abortion with mifepristone and misoprostol is recommended by the World Health Organization as a safe method in early pregnancy. Serious complications are uncommon, and most can be treated when caught early.

Safety depends on three things: genuine medicines, a medical assessment beforehand (including ruling out an ectopic pregnancy), and follow-up afterwards. The biggest dangers come from counterfeit pills and from going without medical support.

Common Side Effects of Abortion Pills

These are expected and are part of how the medicines work. They are usually strongest on the day misoprostol is taken.

Side effectWhat to expectComfort tips
CrampingMild to strong, like intense period painHeating pad, rest, pain relief your clinician recommends
BleedingHeavier than a period, with clots, then lighterUse large pads, not tampons
Nausea or vomitingUsually within hours of misoprostolSmall sips of water, light food
DiarrhoeaShort-livedDrink plenty of fluids
Chills or low feverCommon on misoprostol dayShould settle within 24 hours
Headache, tiredness, dizzinessTemporaryRest and stay hydrated

How Long Do Abortion Pill Side Effects Last?

  • Strong cramping and heavy bleeding: usually ease within 24 hours of misoprostol.
  • Nausea, chills and diarrhoea: usually settle within a day.
  • Light bleeding or spotting: can continue on and off for 2–4 weeks.
  • Tiredness: may last a few days.

Possible Complications

These are less common but important to know about.

Incomplete abortion

Some pregnancy tissue stays in the uterus and may need more medicine or a short procedure.

Continuing pregnancy

The medicines do not end the pregnancy. This is rare in early pregnancy.

Heavy or prolonged bleeding

Bleeding that is excessive or does not ease as expected.

Infection

Uncommon, but needs prompt treatment.

Allergic reaction

Rare. Swelling of the face or difficulty breathing is an emergency.

Your individual risk depends on your medical history, how far along the pregnancy is, and the treatment used.

Warning Signs: When to Get Urgent Medical Care

Go to a hospital immediately if you have:

  • Bleeding that soaks two or more large pads an hour for two hours in a row
  • Severe or worsening abdominal pain that pain relief does not help
  • Fainting or severe dizziness
  • A fever of 38°C or higher lasting more than 24 hours after misoprostol
  • Foul-smelling vaginal discharge
  • Severe pain on one side, or shoulder-tip pain (possible ectopic pregnancy)
  • Difficulty breathing or swelling of the face or throat
Ambulance: 997MOH helpline: 937

Contact a clinician soon if:

  • You have no bleeding within 24 hours of misoprostol
  • Vomiting, diarrhoea or weakness continue beyond 24 hours
  • Pregnancy symptoms continue after a week

What If the Abortion Pill Doesn’t Work Completely?

Medication abortion is highly effective but not 100%. Sometimes the pregnancy continues or tissue remains. Bleeding and cramping alone cannot confirm the abortion is complete.

A clinician can check with a specific pregnancy test, a blood test or an ultrasound, and may recommend more medicine or a short procedure. Do not take extra doses on your own. See how effective the abortion pill is, and our guides to using a pregnancy test and reading the results.

Does the Abortion Pill Have Long-Term Side Effects?

For most people, no. An uncomplicated medication abortion is not linked to long-term health problems.

Future fertility

It does not generally affect fertility. Ovulation can return within weeks, so pregnancy is possible before your next period. Discuss contraception if you want to avoid this.

Future pregnancies

An uncomplicated medication abortion is not generally associated with a higher risk of miscarriage, ectopic pregnancy, premature birth or low birth weight later. A serious untreated infection can affect future health, which is why warning signs need prompt care.

Breast cancer

Current medical evidence does not show that abortion increases breast cancer risk, according to ACOG.

Emotional wellbeing

Feelings afterwards vary, from relief to sadness or a mix of both. These feelings are not a medical complication. If distress continues or affects daily life, a counsellor or doctor can help.

Who Needs Extra Medical Care Before Taking Abortion Pills?

Medication abortion is not suitable for everyone. A clinician should assess you first, especially if you:

  • Have a suspected or confirmed ectopic pregnancy (abortion pills do not treat it)
  • Have a bleeding disorder or take blood-thinning medicines
  • Have an IUD (coil) in place
  • Have severe anaemia, or adrenal, heart or liver conditions
  • Have had an allergic reaction to mifepristone or misoprostol
  • Are unsure how far along the pregnancy is

Abortion Pill Safety in Saudi Arabia

  • Counterfeit pills are the biggest risk. Pills sold online or through WhatsApp are often fake, the wrong strength, or contain nothing at all. See how to spot fake pills.
  • Regulated medicines: abortion medicines are regulated by the Saudi Food and Drug Authority (SFDA) and are not sold freely.
  • Legal access is restricted: abortion is generally permitted only for medical reasons through authorized healthcare channels. See our legal overview.
  • Emergency care: if you have warning signs, go to the nearest hospital emergency department or call 997. Doctors are there to treat you.
